Ok! So now we have a nice shiny list of what makes what :O, now I bet you're wondering "But where do I get my shards! :(", Fear not my friends! Here is another list! (Yaaaay lists!)
Red Matrix Shard Locations: Ord Mantell (Republic) - Coordinates: 770, 134 - Located on the beach on the north side of Mannett Point
Korriban (Imperial) - Coordinates: -54, 379 - Located in the Tomb of Tulak Hord area
Hoth - Corrdinates: -738, 1705 - Located in the southeastern part of Highmount Ridge at the back of an ice mountain/hill. You must purchase a Hydro-Thinner from the Cantina vendor in your faction's base for 18,000 credits in order to thaw out the datacron from its ice enclosure. *NOTE* If you are doing this in a group, only one person needs the Hydro-Thinner.
Ilum - Corrdinates: 543, 544 - Located above the Republic Base inside a tent.
Ok so now we have all the shards listed, what do we do with these shards?! :O No, no, they are not edible (I tried).
For Republic, head to the Jedi Temple on Coruscant, take the elevator to the the ruins, take a right off the elevator. You should see the green instance portal.
For the Empire, Head south of the temple on Dromund Kaas
So now we've got everything? As Bench would say: Not so fast, hotpants! What happens if you are not happy with your chosen relic? Are your shards wasted forever…?

Believe it or not, by the power of Jawa (literally!), there is a device that can dismantle your relics! Nifty!
Check out the Junk Vendors on Tattooine, Taris and Balmorra. The vendor will sell a Code cylinder, you can buy this for 8,000 creds and it'll open the chest that's right next to them and inside said chest is your disassembler-gadget thingy! Aaaand I think that's all there is to making cubes? Haha :D
